I attest that I am authorized to use the payment method provided as the cardholder, authorized user on the cardholder’s account or owner of the account being used.

I understand that this fee is strictly for the agreed upon locations and inspections. This fee does not include cell phones, laptops, or computers. An additional fee will be incurred should I wish to have any other areas or items inspected or should the technician be needed for any future hearings, depositions, court proceedings, or anything having to do with the inspection. I also understand that there is always a possibility the results of this inspection may reveal that on the date and time of the inspection there may be no evidence of any electronic eavesdropping devices present. I acknowledge that the technician cannot, and does not, guarantee the presence of any unauthorized bugging or tracking devices.
